Cleaved Lymphocytes in Peripheral Blood Smear Clues for Rapid Diagnosis of Bordetella Pertussis

A 23-day-old infant presented with cough and dyspnea. Peripheral blood smear revealed marked leukocytosis with numerous cleaved lymphocytes (14% of leukocytes), prompting suspicion of pertussis. Myeloperoxidase staining confirmed their lymphoid origin. Bordetella pertussisinfection was confirmed by PCR, leading to a diagnosis of whooping cough. The patient recovered after treatment. This case underscores the diagnostic value of cleaved lymphocytes in peripheral blood smears for early suspicion of pertussis, particularly in young infants. Further studies are needed to define their diagnostic threshold and immunophenotype.
