Oral and Maxillofacial Surgery (OMS) has been a pioneer in the three-dimensional planning of surgical procedures based on DICOM data and in the use of these planning datasets for the fabrication of patient-specific implants. Although three-dimensional virtual surgical planning and its implementation using surgical guides and/or patient-specific implants are not mandatory standards of care in every case, they have become indispensable in the management of complex craniofacial surgery. The predictability of even the most challenging reconstructions and secondary corrective procedures has reached an entirely new level. Nevertheless, the practical feasibility of translating the virtual plan into the surgical setting must always be critically assessed.Using selected clinical examples of secondary corrective procedures, this presentation will illustrate the capabilities of virtual surgical planning and its implementation with patient-specific surgical guides and implants.It is therefore all the more remarkable that the adoption of these digital planning technologies and their translation into patient-specific guides and implants remains comparatively hesitant in many other surgical specialties. Oral and Maxillofacial Surgery has once again demonstrated its longstanding role as an early adopter of digital innovations, continuing a tradition that has characterized the specialty for decades.
