<p>With the rapid development of artificial intelligence (AI) applications in recent years and the emergence of large language models such as ChatGPT, the question arises as to how oncologic reconstructive oral and maxillofacial surgery can benefit from this development. Based on the relevant challenges in current treatment, this article presents possible approaches for digitalization, especially with AI, including deep learning, radiomics, pathomics, digital twins, augmented reality, virtual reality, robotics, clinical prediction models, process mining, and large language models. Finally, barriers to the use of AI, including ethics, translation, and benefits, will be discussed. Collaboration with medical informatics, including clinical informaticians, could provide important impetus, as the challenges of digital transformation can only be met through interdisciplinary collaboration across disciplinary boundaries. Oral and maxillofacial surgery should not leave this development to chance or even to third parties, but should take the reins and actively shape it in the interest of its patients.</p>
