Total alloplastic temporomandibular joint (TMJ) replacement (TRJ) is a procedure that has been increasingly used in advanced TMJ diseases over the past 20 years. In general, the fossa and the condyle are replaced. The main indications are end-stage osteoarthritis, traumatic, inflammatory or tumor-related loss of one or both temporomandibular joints and ankylosis. Treatment-resistant arthropathies are also an indication for TRJ. In children, TRJ can be indicated particularly in cases of syndromic craniofacial diseases involving the temporomandibular joint. The main contraindications are allergies and hypersensitivity to components of the metal alloys in the implant. In general, TRJ has a high success rate in terms of improving quality of life and a low complication rate. The implants are designed for lifelong wear.
