Total Gland Necrosis in Acute Necrotizing Pancreatitis : A Case Report of Survival

Acute necrotizing pancreatitis (ANP) with complete gland necrosis carries a high mortality rate, especially when infected. Multidisciplinary, step-up approaches have redefined survival prospects. We report a case of a 44-year-old male with diabetes and alcohol use disorder who developed total pancreatic necrosis. His recovery involved multiple surgical debridements, angioembolizations, ventilator support, and prolonged hospitalization. Despite three major complications—including splenic and gastroduodenal artery pseudoaneurysms—he survived with complete pancreatic loss and achieved functional recovery. This case underscores the importance of structured step-up care, timely decision-making, surgical judgment, and multidisciplinary coordination in managing total pancreatic necrosis.
