Primary Duodenal Carcinosarcoma: A Rare Biphasic Malignancy

Carcinosarcoma is a rare and aggressive malignancy composed of both epithelial and mesenchymal components. Although it has been described in various organs, primary involvement of the duodenum is exceedingly rare. We report a 30-year-old female presenting with vomiting and weight loss. Imaging revealed a large duodenal mass with metabolic activity. She underwent open pancreaticoduodenectomy with en bloc resection of involved structures. Histopathology demonstrated a biphasic neoplasm composed of adenocarcinoma and sarcomatous elements, confirmed by immunohistochemistry. Duodenal carcinosarcoma is an aggressive tumor with limited reported cases. Radical surgical resection remains the cornerstone of management, and close postoperative surveillance is mandatory due to the high risk of recurrence.
