Urachal Actinomycosis: The Elusive Mimic of Bladder Cancer Unveiled

Urachal actinomycosis is a rare infectious entity that often mimics urachal malignancy due to its infiltrative growth pattern and mass-forming presentation. We report a case of primary urachal actinomycosis presenting as a malignant-appearing mass on the bladder dome. Preoperative imaging suggested suspected urachal adenocarcinoma, and the diagnosis was established only after surgical excision and histopathological examination. This case highlights the importance of considering actinomycosis in the differential diagnosis of urachal masses to avoid overtreatment and to guide appropriate antimicrobial therapy.
