<p>Gallbladder cancer is a common malignancy of biliary tract that often presents at an advanced stage due to its aggressive behaviour and is associated with poor prognosis and reduced survival. It most frequently metastasizes to the liver and regional lymph nodes, while metastasis to the breast is exceedingly rare. Here, we report an extremely rare case of synchronous gallbladder carcinoma with breast metastasis in a 60-year-old female, confirmed by histopathology, immunohistochemistry, Contrast Enhanced Computed Tomography scan (CECT) and tumour marker evaluation. Owing to the typically poor prognosis and a median survival of less than one year, management is mainly palliative, with emphasis on symptom relief and improvement in quality of life. In the present case, the patient showed a favourable response to palliative chemotherapy with gemcitabine and cisplatin, achieving symptomatic improvement, and an overall survival of 14 months from the initiation of treatment. Literature describing gallbladder carcinoma with metastasis to the breast remains extremely limited.</p>
