Clinicopathological Profile and Outcome of Mammary Paget’s Disease with and Without Invasive Breast Cancer in a Tertiary Referral Center in a Developing Country

Introduction: There is a scarcity of literature on Mammary Paget’s disease (MPD) from developing countries. This study aimed to investigate clinicopathological features and outcomes of MPD in patients with and without invasive cancer (IC). Patients and Methods: Retrospective study (January 2000- December 2022) consisting of 53 patients with MPD undergoing surgery in a tertiary referral center. Clinicopathological profile and outcomes were compared between Group 1- MPD with IC; and Group 2- MPD without IC. Results: The incidence of IC was 77.4%, and ductal carcinoma in situ with or without IC was 52.8% in the cohort. Overall, early, locally advanced and metastatic disease was observed in 66, 26.4, and 7.5% of patients, respectively. The incidence of human epidermal growth factor receptor- 2 positive tumors was 68.1% and hormone receptor-positive tumors was 23.4%. Mastectomy, breast conservative surgery (BCS) and sentinel lymph node biopsy were performed in 88.7, 11.3 and 26.4% of patients, respectively. Radiotherapy, chemotherapy and hormonal therapy were administered in 71.6, 68 and 18.9% respectively. Clinicopathologic profile of Group 1 and Group 2 was not significantly different. The number of patients undergoing BCS was higher albeit non-significantly in Group 2 but significantly more in Group 1, received radiotherapy (p = < 0.001). The overall survival (p = 0.533) and disease-free survival (p = 0.115) were not significantly different. On univariate analysis, OS in Group 1 was significantly associated with molecular type (p = 0.026) and tumor stage (p = 0.013). Conclusions: The majority of MPD patients presented with IC but BCS is still feasible in select few.
