<p>Cysto-biliary communication is an uncommon complication of hepatic hydatid cysts. It requires accurate identification, as percutaneous procedures such as PAIR are contraindicated, and surgery is considered the definitive management. We present the case of a 52-year-old male with frank rupture of hepatic hydatid into the biliary tree with characteristic imaging findings, who was managed surgically.</p>
