<p><i>Aspergillus</i> species, as opportunistic pathogens, primarily invade the lungs and are relatively rare in causing brain abscesses. <i>Aspergillus</i> brain abscess is a severe infectious disease with nonspecific clinical manifestations, making its diagnosis challenging. This case reports a 74-year-old healthy male patient who was urgently admitted to the hospital after drowning in a public bath. During treatment, the patient lost consciousness and had respiratory failure. Through bacterial and fungal cultures, GM test, and NGS, the patient was ultimately diagnosed with multiple pulmonary infections and <i>Aspergillus</i> brain abscess. This case provides valuable clinical experience in the diagnosis and treatment of <i>Aspergillus</i> brain abscess in immunocompetent patients following drowning.</p>
