Serum C-reactive protein levels on postoperative day 3 as a predictive factor for both short- and long-term outcomes after gastric cancer surgery

The relationship between inflammation and cancer has been generally accepted. This study aimed to investigate the utility of serum C-reactive protein (CRP) level measured on a specific day During the early postoperative period as a predictor of short- and long-term outcomes after gastric cancer surgery. The clinicopathological data of 636 patients who underwent gastrectomy for gastric cancer were reviewed. The relationship between the laboratory data including serum CRP levels on postoperative day 3 (CRP3) and short- and long-term outcomes was investigated. A total of 133 patients (20.9%) developed Clavien-Dindo (CD) ≥ grade 2 complications postoperatively. The area under the receiver operating characteristic curve of CRP3 for postoperative complications was 0.759. The univariate and multivariate analyses showed that a CRP3 of ≥ 14.01 mg/dL was a significant predictor. The 5-year overall survival (5y OS) rate in the high CRP3 group (≥ 12.0 mg/dL) was significantly worse than that in the low CRP3 group (< 12.0 mg/dL) (71.5% vs. 81.8%, p = 0.003). In the stage-matched analysis, the high CRP3 group showed worse 5y OS than the low CRP3 group (53.5% vs. 68.6%, p = 0.032), and multivariate analysis showed that CRP3 was a significant prognosticator (p = 0.044) in pStage II/III gastric cancer. However, no significant difference was found between the two groups among those with pStage I gastric cancer (p = 0.700). In conclusions, CRP3 is a readily available and useful predictive factor for both short- and long-term outcomes after surgery for gastric cancer, especially in patients with pStage II/III disease.
