<p>Intestinal obstruction caused by primary common bile duct stones in patients with a history of cholecystectomy is extremely rare. The term <i>gallstone ileus</i> is a misnomer and typically refers to a mechanical intestinal obstruction resulting from the impaction of one or more gallstones within the gastrointestinal tract. These stones most commonly lodge in the terminal ileum due to its narrow lumen. We present a first of its kind case of a 90-year-old lady who presented with complaints of colicky abdominal pain, multiple episodes of bilious vomiting, and constipation lasting for 1&#xa0;week. Imaging studies revealed a markedly dilated common bile duct, extrahepatic biliary radicle dilatation, and multiple dilated bowel loops. Pneumobilia was also noted, suggesting the possibility of gallstone ileus. A diagnosis of intestinal obstruction secondary to primary gallstones was established, and surgical intervention was planned to relieve the obstruction. Incidence of primary choledocholithiasis after cholecystectomy is an extremely rare entity. This first of its kind case underscores the importance of considering primary CBD stone causing intestinal obstruction as a differential diagnosis for small bowel obstruction, especially in elderly patients.</p>
