<p>Distal cholangiocarcinoma (dCCA) is a rare malignancy arising from the epithelial cells of the distal biliary tract and has a poor prognosis. Surgical resection being the only potential curative treatment, outcomes remain poor, especially in advanced cases. We report a case of a 66-year-old woman with the distal cholangiocarcinoma who developed severe postoperative complications, including portal vein stenosis and thrombosis after a Whipple procedure. Following portal vein stent placement, the patient underwent anticoagulant therapy but experienced complications, including migrated portal vein stents and gastrointestinal hemorrhage. Surprisingly, after 3&#xa0;years of follow-up, the patient’s tumor did not recur. We are guessing that acute local inflammation due to the displaced portal stent contributed to sustained anti-tumor immune responses, preventing tumor recurrence. This case highlights the potential role of acute inflammation in suppressing tumor progression, offering insights into novel therapeutic approaches for CCA management.</p>
