<p>The aim of our case series analysis was to assess the feasibility, safety, and efficacy of the Desarda tissue-based hernia repairs in emergency setting with long-term follow-up. A retrospective evaluation was conducted on all patients who underwent emergency Desarda hernia repair. Thirty-two patients underwent complicated primary inguinal hernia repair using the Desarda technique by surgeons experienced in abdominal wall surgery over a 5-year period between January 2016 and April 2020. The feasibility and safety of the procedure were evaluated based on the outcomes as operative time and complications. The effectiveness of the procedure was assessed by recurrence rate and chronic pain during the follow-up period. There were 5 women and 27 men with a mean age of 65.7&#xa0;years (range 50 to 87&#xa0;years). During a mean follow-up period of 58&#xa0;months, no chronic pain was observed. We detected a patient with recurrence 14&#xa0;months after surgery. The Desarda technique for primary inguinal hernia in the emergency setting is safe, feasible, and can be an effective therapeutic option when the use of mesh is not preferred.</p>
