Endobronchial PEComa of the Lung: Atypical Presentation and Surgical Approach

A rare case of a perivascular epithelioid cell tumor (PEComa) presenting as a post-obstructive pneumonia syndrome, with hypermetabolic findings on PET/CT, is reported. Successful resolution was achieved following a video-assisted thoracoscopic surgery (VATS) lobectomy. Bronchoscopy revealed an endobronchial tumor with a morphology resembling a carcinoid tumor. While pulmonary localization of PEComa is uncommon, and high uptake on dynamic studies is rare, this case highlights an atypical presentation. No previous report of endobronchial presentation of PEComas has been found.
