Early Postoperative Prosthesis for Below-Knee Amputee: An Innovative Technique and Design (Siachen Hospital EPOP)

Amputation is a devastating experience. Postoperative management strategy can achieve restoration of function and quality of life. The standard approach includes—removable, non-removable, soft, semi-rigid, rigid dressings or early postoperative prostheses. These all aim to reduce oedema, protect from external trauma and prevent knee contracture. The early interim prostheses facilitate early ambulation, musculoskeletal reconditioning, acceptance of definitive prosthesis and rehabilitation. Interim prostheses result in better outcome but are not frequently used because of lack of familiarity, skill and local practices and may require multiple change of cast for frequent monitoring of wound. Moreover, the cost and the availability of rehabilitation expertise preclude its use. This article aims to provide an innovative design and method of fabrication of a removable early postoperative prosthesis that is safe and simple and has advantage over the conventional dressings for below-knee amputee. Siachen Hospital’s early postoperative prosthesis was designed and custom made for 12 cases of below-knee amputee at a low cost, and the experience is being shared. All these patients were ambulant by 3rd–20th postoperative day and had early rehabilitation.
