<p>Intramedullary nailing for clavicle midshaft fractures is advantageous in reducing complications, but it has limitations in rotational stability. This study compared the fixation stability of anterograde and retrograde intramedullary nailing techniques for clavicle fractures. A three-dimensional clavicle model was created from CT images of a human left clavicle. Three kinds of transverse clavicle midshaft fractures were modeled as located at the central, medial, and lateral to the clavicle center. Under the bending, anterograde and retrograde nailings showed no significant difference in displacement and stress. Under the torsion, the retrograde nailing exhibited a significantly larger in-plane rotation and a larger slip than the anterograde nailing. Under the torsion, the overall rotation and slip were greater in retrograde nailing. The anterograde nailing would have better rotational stability for the fixation of the clavicle medial midshaft fracture, compared with the retrograde nailing.</p>
