Electrical cardioversion (ECV) is an established and effective method for restoring sinus rhythm in patients with persistent atrial fibrillation and should always be part of a rhythm-maintaining holistic concept. This position paper by the German Cardiac Society (DGK) provides practical, evidence-based recommendations to guide the safe and effective clinical implementation of ECV. Key topics include the indications, patient-specific risk factors, preparation and framework conditions (including anticoagulation and possibly transesophageal echocardiography), technical aspects (e.g., electrode selection, the shock pathway and energy levels, sedation), approaches in nonconvertible patients, early recurrences, and complications of ECV. Specific recommendations are given for ECV in high-risk groups, including patients with obesity, pregnancy or cardiac implantable electronic devices (CIED). The recommendations are based on the current evidence and expert consensus and are aimed at a standardized and safe execution. In this way the quality of care should be sustainably improved.
