DGK-Kommentar zu den Leitlinien der ESC (2024) zur Behandlung der peripheren arteriellen und aortalen Erkrankungen

The updated European Society of Cardiology (ESC) guidelines on peripheral arterial and aortic diseases (PAAD) summarize comprehensive recommendations for the diagnostics, treatment and follow-up of these diseases of the arterial vascular system. For the first time PAAD are considered as a single entity that includes not only aortic diseases but also peripheral arterial occlusive disease of the lower extremities (PAOD) and stenoses and occlusions of the visceral and renal arteries, the brachial arteries and the extracranial vessels supplying the brain. Despite their high prevalence and considerable morbidity and mortality, these diseases often remain underdiagnosed and undertreated. Particular innovations include extended diagnostic recommendations and optimized drug treatment options, especially in the areas of antithrombotic treatment, lipid and blood pressure therapy and diabetes management. The guidelines also contain differentiated recommendations for action in interventional and surgical treatment, which are supplemented by new classification systems for individual risk stratification. A central focus is on a multidisciplinary care approach in specialized high-volume and competence centers. At the same time, shared decision-making with patients is emphasized in order to enable individually tailored treatment planning.
