<p>In August 2024 the updated guidelines of the European Society of Cardiology (ESC) on the management of atrial fibrillation were published. Atrial fibrillation remains the most frequent sustained arrhythmia and still has a relevant use of resources in primary and secondary prevention in the healthcare system. The concise and easy to read guidelines focus on evidence-based recommendations and practical implementation including clear flow charts and treatment pathways. Holistic care of patients and their comorbidities is given priority and integrated into a&#xa0;comprehensive, interprofessional AF-CARE concept. The topics are grouped intuitively around the CARE acronym, C for comorbidities and risk factor management, A for avoidance of stroke and thromboembolism, R for reduction of symptoms by rate and rhythm control in anticipation of E for evaluation and dynamic reassessment at the follow-up. There are innovations in the prioritization of rhythm control: in paroxysmal atrial fibrillation ablation is now first-line treatment. Current data for device-detected atrial fibrillation have been integrated into the guidelines. A maximum involvement of patients, caregivers and others by e.g. patient guidelines is promoted for mutual informed decision making. The implementation should now be carried out according to the national and regional situations to ensure optimal management of atrial fibrillation patients in an AF-CARE network. This article discusses the essential changes and recommendations from the perspective of the German Cardiac Society (DGK).</p>
