<p><sup>225</sup>Ac targeted alpha therapy is characterized by high linear energy transfer (LET) and short range, which has demonstrated outstanding therapeutic effects and minimal adverse effects in the treatment of prostate cancer and neuroendocrine tumor. This review paper aims to summarize the research conducted to date on <sup>225</sup>Ac targeted alpha therapy applied to the treatment of patients with prostate cancer and neuroendocrine tumor. Efforts has been made to improve the therapeutic outcomes of <sup>225</sup>Ac targeted alpha therapy by exploring new protocols, and its application has been expanded to treat other types of tumors beyond these disease entities. Although numerous studies have reported the excellent therapeutic effects of <sup>225</sup>Ac targeted alpha therapy, further clinical research is required for it to become established as a standard clinical treatment.</p>
