Psychological health survey of periodontitis patients in the post-pandemic period: analysis of factors affecting anxiety, depression, and stress

Periodontal disease is one of the most prevalent oral health problems worldwide which causes a substantial impact on overall well-being. This study investigates the anxiety, depression, and stress levels in periodontitis patients and identifies influencing factors, providing evidence for clinical and psychological interventions. The study included 191 periodontitis patients from the Dental Hospital of Changzhou Traditional Chinese Medicine Hospital (May 2024-March 2025) and 187 healthy controls. Participants were surveyed using a self-designed general questionnaire, the Depression Anxiety Stress Scales-21 (DASS-21), Oral Health Self-Efficacy Scale (SESS), and the Short Version Oral Health Literacy Scale (HeLD-14). Stress, anxiety, and depression were more prevalent in periodontitis patients (11.5%, 32.5%, and 27.7%, respectively) compared to controls (P < 0.05). Multivariate logistic regression revealed that severe periodontitis (OR = 3.066), being unmarried (OR = 4.836), and being divorced (OR = 4.888) were risk factors for stress. Higher HeLD-14 (OR = 0.926) and SESS (OR = 0.947) scores were associated with lower odds of stress. Severe periodontitis (OR = 3.462) and being unmarried (OR = 3.906) were associated with higher odds of anxiety, while higher HeLD-14 (OR = 0.936) and SESS (OR = 0.938) scores were associated with lower odds of anxiety. Severe periodontitis (OR = 3.296), being unmarried (OR = 4.390), and being divorced (OR = 4.479) were associated with higher odds of depression, with higher HeLD-14 (OR = 0.933) and SESS (OR = 0.942) scores being associated with lower odds of depression. Anxiety, depression, and stress are significantly more prevalent in periodontitis patients. Marital status, periodontitis severity, and oral health literacy show significant associations with psychological health, highlighting the need for mental health support and personalized treatment plans.
