Efficacy and safety of acupuncture in the treatment of post-traumatic stress disorder: A Systematic Review and Meta-analysis

Although previous studies have preliminarily explored acupuncture’s therapeutic potential for post-traumatic stress disorder (PTSD), systematic quantitative syntheses of its intervention efficacy remain limited. This meta-analysis aimed to comparatively evaluate the efficacy, safety, and acceptability of acupuncture versus pharmacotherapy, psychotherapy, and waitlist controls in adult PTSD populations. We systematically searched 8 databases (PubMed, Embase, Web of Science, Cochrane Library, PsycINFO, CNKI, Wanfang, CBM) for randomized controlled trials (RCTs) investigating acupuncture for PTSD published before October 1, 2024. PTSD severity served as the primary outcome, with depression, anxiety, adverse events (AEs), and dropout as secondary outcomes. Continuous outcomes were analyzed using standardized mean differences (SMDs), while dichotomous variables were assessed via relative risk with 95% confidence intervals (CIs). We assessed heterogeneity using I², τ², and χ² statistics. Methodological quality was appraised using the Cochrane Risk-of-Bias Tool 2.0, while evidence certainty was graded via GRADEpro. Publication bias was examined through funnel plot asymmetry and Egger’s regression test. Sixteen RCTs (N = 1,168 participants) met inclusion criteria. Compared to the control group, acupuncture demonstrated large effect sizes in reducing PTSD(SMD=1.23, 95% CI: 0.81 to 1.64, P<0.001), depression (SMD=0.97, 95%CI: 0.42 to 1.52, P<0.001) and anxiety (SMD=1.15, 95%CI: 0.97 to 1.34, P<0.001) with substantial heterogeneities (I2 > 90%, P<0.001). No significant between-group differences emerged in AE incidence or dropout rates. Despite detectable publication bias (Egger’s test P=0.005), GRADE assessments rated evidence certainty as high for all outcomes. Our findings provided preliminary evidence that acupuncture might constitute an effective and well-tolerated intervention for PTSD and comorbid affective disorders.
