<p>This study aims to systematically evaluate the effects of mindfulness-based interventions on psychological emotions, quality of life, and pregnancy outcomes in patients undergoing in vitro fertilization-embryo transfer (IVF-ET). A comprehensive search was conducted across PubMed, Web of Science, Embase, Cochrane Library, CINAHL, CNKI, Wanfang, VIP, and CBM from their inception to November 18, 2024. The Cochrane risk-of-bias tool was used to assess the risk of bias, and meta-analysis was performed using RevMan 5.3 and Stata 15.0. Thirteen randomized controlled trials meeting the inclusion criteria were analyzed. The meta-analysis results showed that, compared to the control group, mindfulness-based interventions significantly improved anxiety (pooled SMD = -1.66, 95% <i>CI</i> [-2.28, -1.03], <i>p</i> &lt; 0.001), depression (pooled SMD = -1.48, 95% <i>CI</i> [-2.02, -0.94], <i>p</i> &lt; 0.001), and stress levels (pooled SMD = -2.15, 95% <i>CI</i> [-2.79, -1.52], <i>p</i> &lt; 0.001), while enhancing quality of life (pooled SMD = -1.21, 95% <i>CI</i> [-2.40, -0.01], <i>p</i> &lt; 0.001). Current evidence indicates that mindfulness interventions have a positive impact on IVF-ET patients. Future studies should prioritize high-quality, multilingual research, employ standardized measurement tools, and provide detailed data. Incorporating long-term follow-up plans is also recommended to validate these findings.</p>
