The effectiveness of motivational enhancement therapy on reducing symptoms severity in individuals experiencing pathological internet use: a systematic review of clinical trials

Pathological internet use (PIU), characterized by uncontrollable internet use, is often referred to by various labels, such as internet addiction disorder (IAD). It is considered a prevalent technological prosperity-related issue that affects users’ physical and psychological health and warrants specialized treatment. However, there is a scarcity of evidence highlighting promising psychotherapeutic modalities, such as motivational enhancement therapy (MET), for PIU treatment. This study aimed to evaluate the effectiveness of motivational enhancement therapy (MET) via the motivational interviewing (MI) technique alone or in combination with cognitive behavioral therapy (CBT) in reducing symptom severity among individuals experiencing PIU. This systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) protocol. Four databases (i.e., CINAHL, ScienceDirect, PubMed, and Google Scholar) were searched for relevant studies published between 2018 and 2024 in the English language. Article screening, data extraction, and quality assessments were performed according to the Cochrane guidelines. A total of nine published studies that met the inclusion criteria were included in this review; seven were RCTs, and two were quasi-experimental. This systematic review presents some supportive evidence focused on the effect of employing MET, mainly as a psychological intervention, in reducing the severity of symptoms of various types of PIU (i.e., general, gaming, and social media). This review highlights the pressing need to develop a standardized protocol for using MET via MI alone or in combination with CBT to manage PIU.
