Prevalence and associated factors of suicide attempts among early and late-onset major depressive disorder patients with obesity

Suicide attempts (SA) represent a critical concern in Major Depressive Disorder (MDD). The high comorbidity of obesity with MDD presents a complex clinical scenario, as obesity-related factors may independently contribute to suicidality. Furthermore, the age of MDD onset is a key stratifying factor linked to different etiologies, symptom severity, and long-term prognosis. However, there remains a significant gap in understanding the prevalence of SA and its contributing factors specifically among MDD individuals with obesity when stratified by early-onset and late-onset subtypes. This study was designed to establish the SA prevalence among obese MDD patients, identify associated contributing factors, and perform comparative analyses between early- and late-onset MDD patients. The study population comprised 1,718 patients with confirmed MDD diagnoses, and both their demographic information and clinical data were gathered. A variety of biochemical parameters, including fasting blood glucose (FBG), lipids and thyroid hormone levels were determined. The 17-item Hamilton Depression Rating Scale (HAMD-17), the Hamilton Anxiety Rating Scale (HAMA), the Positive and Negative Syndrome Scale (PANSS) positive subscale, and the Clinical General Impression of Severity Scale( CGI)were utilized to measure clinical symptoms. Prevalence of MDD patients with obesity was 59.5% (1023/1718), the SA rate was 18.6% (142/763) among early-onset MDD patients with obesity, and the prevalence of SA was 22.7% (59/260) for late-onset MDD patients with obesity. No statistically significant difference in SA prevalence was observed (p > 0.05). Univariate analysis showed that gender, education, psychotic symptoms, illness duration, HAMD and HAMA scores, PANSS positive score, CGI, TSH, A-TPO, A-TG, FBG, TC, LDL-C, HDL-C, SBP, and DBP levels demonstrated a significant correlation with suicide attempts among early-onset MDD patients with obesity. Psychotic symptoms, HAMD and HAMA scores, PANSS positive scores, CGI, TSH levels, A-TPO, A-TG and FBG level, TC, SBP, DBP levels demonstrated a significant correlation with suicide attempts among late-onset MDD patients with obesity. Logistic regression modeling confirmed that female, HAMA score, PANSS positive score, A-TPO, CGI and systolic BP levels were independent factors among early-onset MDD patients with obesity. PANSS positive score, A-TPO, CGI, and diastolic BP levels were independent factors among late-onset MDD patients with obesity. All of the study subjects were Han Chinese patients. The cross-sectional design precludes causal interpretation of observed associations between risk factors and SA in obese MDD patients. The present investigation identified a significantly higher obesity rate in patients with MDD. It also points out that the influencing factors for SA, including various clinical indicators and psychiatric symptoms. Future research can develop targeted interventions and preventive measures based on these findings.
