Reliability and validity of the Chinese version of subjective cognitive decline questionnaire in middle-aged and elderly people

Objectives. Subjective cognitive decline (SCD) has received attention in the field of Alzheimer's disease (AD) as it can indicate the preclinical stage of AD. Developing simple and effective screening tools for early AD detection is important. This study aimed to validate the Chinese version of the Subjective Cognitive Decline Questionnaire (SCD-Q) and determine its cut-off score for distinguishing individuals with SCD from a cognitively unimpaired group, providing a reliable tool for early SCD identification in China. Methods. A total of 380 participants aged ≥ 45 years were recruited. Psychometric analyses, including item analysis, internal consistency reliability (McDonald's ω), exploratory factor analysis (EFA), and confirmatory factor analysis (CFA), were conducted. Logistic regression and receiver operating characteristic (ROC) analysis were used to examine affecting factors and diagnostic ability. Results. SCD-Q showed good item discrimination and homogeneity. Internal consistency reliability was high (McDonald's ω = 0.920). EFA supported a four-factor structure (KMO= 0.885), and CFA confirmed the questionnaire's validity (RMSEA= 0.029, CFI= 0.988, TLI= 0.986.). Logistic regression identified education and insomnia as affecting factors. The SCD-Q demonstrated good diagnostic ability (AUC = 0.878), with an optimal cut-off score of 6. Conclusions. The Chinese version of SCD-Q is a reliable and valid tool for early SCD identification in China.
