<p>The desire to childbearing serves as the primary impetus for deciding to embark on the journey of parenthood or fertility. However, there remains a dearth of understanding regarding how couples navigate the complexities of childbearing. The aim of this study endeavors to elucidate the process through which Iranian couples confront the challenges of childbearing. Employing a qualitative approach and grounded theory methodology, this study was conducted in 2023. The participants consisted of couples residing in the city of Semnan, Iran, who were purposefully and theoretically selected to ensure maximum diversity. Unstructured in-depth and semi-structured interviews served as the primary methods of data collection. Data analysis and collection were carried out iteratively and comparatively, following the method outlined by Corbin et al. (<CitationRef CitationID="CR12">2015</CitationRef>). Data analysis revealed that couples grappled with various challenges influenced by their specific social and cultural contexts, oscillating between the desire to have children and the apprehension of doing so. The decision-making process regarding parenthood often revolved around the “triangle of fear of pregnancy and childbirth” and “parenting anxiety”. This fear and anxiety arose from by various factors, including past experiences of pregnancy and childbirth complications, concerns about loss of sexual attractiveness, challenges associated with child rearing within families, economic insecurity, and apprehensions about the future. In response to these fears and anxieties, couples adopted strategies such as pregnancy avoidance and reliance on faith in God. Furthermore, considerations regarding childbearing, including the choice of birth method, played a significant role in their coping mechanisms. These strategies, while aimed at managing the fear of pregnancy and childbirth and parenting anxiety, ultimately contributed to achieving a sense of comfort and fulfillment in life, juxtaposed with the fulfillment of maternal and paternal instincts. Clarifying this process is a crucial step towards improving the childbearing experience in this region and addressing the challenge posed by declining youth population.</p>
