Objectives <p>Rotavirus gastroenteritis (RVGE) is a major cause of morbidity and mortality in children aged under 5 y in India. This study aimed to describe the prevalence, seasonality, age distribution, clinical characteristics, and genotypes of rotavirus infections in children in Maharashtra, India.</p> Methods <p>This descriptive study was conducted at four centers in Maharashtra from December 2020 to December 2023 as part of a national multicenter surveillance program. Children aged 0–59 mo presenting with acute gastroenteritis were enrolled. Stool samples were tested for rotavirus using an enzyme-linked immunosorbent assay, and positive samples were genotyped. Demographic and clinical data were analyzed using descriptive statistics.</p> Results <p>Of the 534 stool samples tested for rotavirus, 173 (32.7%) tested positive. Of the 529 participants with vaccination records available, 377 (71.3%) had received at least one dose of rotavirus vaccine. In this study, the rotavirus positivity was similar in vaccinated (32.6%) and unvaccinated (32.9%) children and was highest in 2022 (41.4%) and lowest in 2023 (27.6%). The incidence of RVGE was higher in cooler months than in warmer months, and higher in children aged 12–23 mo than in older and younger children. The incidence of vomiting, diarrhea, fever, and severe dehydration was higher in rotavirus-positive than in rotavirus-negative children. Of the genotypes identified, 67.2% were G3P[8] or G2P[4].</p> Conclusions <p>Rotavirus remains a major cause of severe gastroenteritis in children in Maharashtra, with seasonal and demographic variation. This study highlights the importance of ongoing surveillance and tailored vaccination strategies to reduce the disease burden.</p>

Methods
