Objectives <p>Globally, diarrheal diseases are a leading cause of infectious disease morbidity and mortality among children aged under 5 y. Rotavirus is a primary cause of severe diarrhea in children, and India accounts for 22% of rotavirus deaths worldwide. This study aimed to determine the prevalence and distribution of circulating rotavirus genotypes among hospitalized children aged under 5 y in Odisha, India.</p> Methods <p>This prospective hospital-based study was conducted between January and December 2023 at four tertiary care hospitals in Odisha. Stool samples were collected from enrolled children aged under 5 y admitted with acute gastroenteritis and tested for rotavirus antigen at a central laboratory using an enzyme-linked immunosorbent assay (ELISA). The ELISA-positive samples were genotyped using heminested reverse-transcription polymerase chain reaction.</p> Results <p>Of the 365 children enrolled, 288 had their stool samples tested for rotavirus. Rotavirus was confirmed in 67 (23.3%) children, and 66 samples were genotyped. The most prevalent genotypes were G2P[4] (<i>n</i> = 27, 40.9%) and G3P[8] (<i>n</i> = 22, 33.3%).</p> Conclusions <p>These findings highlight the importance of ongoing molecular surveillance to monitor the circulating rotavirus strains to inform public health strategies and future research aimed at optimizing rotavirus vaccination programs.</p>
