Background <p>Permanent radioactive Iodine-125 seed implantation (RISI) is a minimally invasive internal radiation technique that effectively kills tumor cells. This study aimed to investigate the local efficacy and safety of RISI in the treatment of recurrent malignant tumors and to analyze the prognostic factors.</p> Methods <p>We retrospectively evaluated 162 patients with recurrent malignant tumors who received RISI in Zhejiang Provincial People’s Hospital during January 2020 to December 2022. Local control time (LCT), tumor response, particle implantation site, tumor size, needle number, particle activity, particle number, dose parameters, and adverse reactions were analyzed.</p> Results <p>With a median follow-up of 14&#xa0;months, the complete response (CR) rate was 17.9%, partial response (PR) 45.1%, stable disease (SD) 27.8%, and progressive disease (PD) 9.2% in all patients. The median LCT of patients with head and neck metastasis, chest metastasis, abdominal pelvis metastasis, and para-bone metastasis was 11.0, 11.5, 8, and 13&#xa0;months, while median overall survival (OS) was 11, 11.5, 8, and 13&#xa0;months, respectively. Univariate analysis showed that tumor size, postoperative D90 dose, and postoperative D100 dose were factors affecting LCT. Smaller tumor size and higher dose level at the implant site contributed to the significantly longer LCT. The dosimetry results showed that D90, D100, V100, V150, and V200 were consistent before and after the operation. In terms of adverse reactions, except for 5 patients with mild pneumothorax, no other serious adverse reactions were reported.</p> Conclusions <p>The application of CT-guided Iodine-125 implantation in the treatment of patients with advanced malignant tumors demonstrates favorable clinical efficacy, achieving a high rate of LCT while maintaining a superior level of safety and minimal postoperative adverse reactions.</p>
