Speech Perception in Quiet and Noise: Bilateral Basic Versus Unilateral Advanced Hearing Aid Fittings

This study examined speech identification performance in quiet and noise between bilateral basic hearing aids and unilateral advanced hearing aids in adults with bilateral moderate to moderately severe sensorineural hearing loss. Twenty-five Kannada-speaking participants (18–55 years) underwent sentence identification testing in quiet and noise (SNR-50) across three conditions: unaided, bilateral basic hearing aids, and unilateral advanced hearing aid fitted to the ear with higher speech identification scores. In quiet, both aided conditions significantly outperformed unaided listening (p < 0.05), but no significant difference was observed between bilateral basic and unilateral advanced fittings. In noise, bilateral basic hearing aids yielded the lowest (best) mean SNR-50 (2.72 dB), followed by unilateral advanced (3.68 dB) and unaided (5.56 dB) conditions. Bilateral basic hearing aids showed a significant advantage over unaided listening (p < 0.01) with large effect size. Bilateral basic hearing aids offer superior speech-in-noise performance compared to unilateral advanced fittings, highlighting the binaural advantage over premium technology in challenging listening environments.
