<p>We present a rare case of high-grade biphasic synovial sarcoma arising in the right buccal mucosa of an 18-year-old male, who presented with a non-healing ulcer and progressive trismus and was managed with multimodal management with radical surgery followed by adjuvant chemo and radiotherapy. This case complements the clinicopathological profile of head and neck synovial sarcomas reported by Dhal et al., highlighting the exceptional oral cavity subsite involvement, utility of FDG-PET imaging, aggressive surgical approach, and encouraging short-term outcomes.</p>
