<p>Papillary thyroid carcinoma (PTC) commonly presents as a thyroid nodule; however, presentation as an isolated pulsatile neck swelling is rare. We report a 23-year-old female with a painless, pulsatile right lateral neck mass without palpable thyroid swelling or lymphadenopathy. Ultrasonography revealed a highly vascular solid lesion anterior to the right common carotid artery. FNAC suggested PTC, and MRI confirmed involvement of the right thyroid lobe with multiple cervical nodes. The patient underwent total thyroidectomy with central and bilateral neck dissection. Histopathology confirmed solid-variant PTC with bilateral nodal metastases. Thepulsatility was attributed to metastatic lymph node location adjacent to major vessels.PTC should be considered in atypical vascular neck swellings even in absence of clinically evident thyroid disease. Early surgical management offers favorabl eoutcomes.</p>
