<p>Lip cancer is one of the most frequent malignancies of the perioral region and reconstruction remains challenging. We report a consecutive series evaluating the applicability, safety, and outcomes of nasolabial flaps for lip reconstruction. We retrospectively analyzed 66 patients treated for lip malignancies between 2011 and 2024. Demographic, clinical, pathological, and surgical variables were collected. Mean age was 69.4 years and 56% were male. Basal cell carcinoma accounted for 62.1% of cases and squamous cell carcinoma for 37.8%. Reconstructions involved the upper lip in 45 cases, lower lip in 20, and commissure in 1. Complications occurred in 15.1%, including five partial and one total flap losses. Tobacco exposure (<i>p</i> = 0.03) and deepithelialized pedicle use (<i>p</i> = 0.004) were associated with flap necrosis. The nasolabial flap is a reliable and versatile option for upper and lower lip reconstruction with satisfactory functional and aesthetic outcomes. Tobacco exposure appears to increase the risk of flap loss and may justify staged reconstruction in selected patients.</p>
