<p>The pinna, being a sun exposed site is more prone to a variety of pigmented lesions. Melanoacanthoma is a rare benign non-invasive pigmented variant of seborrheic keratosis, characterized by the presence of dendritic melanocytes and epidermal keratinocytes. While it typically presents over trunk, face or scalp involvement of pinna is exceptionally uncommon and poses diagnostic challenges owing to broad differential diagnosis, with only a few cases reported in Indian literature in recent past. We report a case of melanoacanthoma of the left pinna in a 56-year-old male which was excised and reconstructed primarily with post aural local flap. Histologically it mimics melanoma, thus recognizing this benign entity early and prioritizing biopsy would ensure accurate diagnosis and avoid unnecessary aggressive treatment.</p>
