<p>Branchial arch anomalies represent the second most frequent congenital lesions of the head and neck in children. They account for around 20% of all cervical masses, and therefore, must be considered in the differential diagnosis of pediatric neck swellings. In this report we present a case of a 33&#xa0;year old female who presented with the complaint of recurrent discharge from an opening behind left ear since 12&#xa0;years. A provisional diagnosis of a type 1 branchial cleft cyst was made. Complete excision of the tract was done under general anaesthesia. The tract measured approximately 7&#xa0;cm in length. It is rare to find a first branchial cleft cyst in the third decade as seen in our case. In such cases, proper investigations such as USG and MRI should be done to make a proper diagnosis. The surgeon should make sure of the complete excision of the tract to prevent recurrence.</p>
