<p>Tympanosclerosis is a major cause of hearing loss in patients with chronic otitis media. This systematic review and meta-analysis evaluated the effectiveness of stapedotomy in treating hearing loss associated with stapes fixation in tympanosclerosis. A comprehensive search of the PubMed, EMBASE, and Cochrane Library databases was conducted. Eight studies involving 533 patients with tympanosclerosis were included. The primary outcomes were air–bone gap (ABG) and success rate. The meta-analysis showed that stapedotomy significantly reduced ABG values compared with control interventions (mean difference: – 0.80 dB, 95% confidence interval: – 4.53 to 2.94, <i>p</i> &lt; 0.00001, I<sup>2</sup> = 96%). Success rates were also significantly higher with stapedotomy (odds ratio: 3.75, 95% confidence interval: 2.55 to 5.51, <i>p</i> &lt; 0.00001, I<sup>2</sup> = 81%). All included studies were of high quality, as assessed using the Critical Appraisal Skills Programme checklist. These findings indicate that stapedotomy is an effective treatment for hearing loss associated with stapes fixation in tympanosclerosis, demonstrating superior hearing outcomes compared with alternative approaches such as stapes mobilization. However, the high heterogeneity among studies and potential publication bias warrant cautious interpretation. Despite these limitations, this meta-analysis supports stapedotomy as a preferred surgical intervention for eligible patients with tympanosclerosis and stapes fixation. Clinicians should consider stapedotomy while taking into account individual patient factors that may influence surgical success. Further large-scale studies with standardized outcome measures are needed to confirm and strengthen these conclusions.</p>
