<p> Chronic recurrent temporomandibular joint (TMJ) subluxation is difficult tomanage with conventional therapies. This study aimed to evaluate the efficacy andsafety of combined autologous blood injection (ABI) and platelet-rich plasma (PRP) inits management. A prospective interventional cohort study wasconducted at the Department of Oral and Maxillofacial Surgery, in private dentalcollege, between January 2022 and December 2024.Eighteen patients were screened; 15 (8 males, 7 females; mean age32.4 ± 6.8 years) met the inclusion criteria and completed the study. Patients received three sessions of combined intra-articular and peri-capsular ABI +PRP injections at monthly intervals. Outcomes included maximal interincisal opening(MIO), visual analogue scale (VAS) pain scores, subluxation frequency, recurrence,complications, and quality of life (QoL). Assessments were performed at baseline, 1month, and 6 months. Mean MIO decreased from 44.8 ± 3.5 mm at baselineto 38.6 ± 2.9 mm at 6 months (p &lt; 0.05), remaining within functional range. VAS scoresimproved from 7.6 ± 1.1 to 2.1 ± 0.9 (p &lt; 0.05). Dislocation frequency dropped from 3.2± 0.8 to 0.2 ± 0.3 episodes/week (p &lt; 0.01). Thirteen patients (86.7%) wererecurrence-free; two (13.3%) reported reduced but persistent episodes. Transientpreauricular numbness occurred in 3 patients (20%). QoL was rated Excellent in 10 cases, Good in 3, and Fair in 2. Combined ABI + PRP injection is aminimally invasive, safe, and effective treatment for chronic recurrent TMJ subluxation,offering stability, functional preservation, and improved QoL. Larger randomized trialswith extended follow-up are warranted.</p>
