<p>Rhinoplasty is one of the most commonly performed aesthetic procedures. Oedema and ecchymosis are common after osteotomy. Bromelain is a pineapple extract that is strongly associated with anti-edema and anti-inflammatory processes. This study aims to investigate the effects of bromelain use on edema and ecchymosis in the early postoperative period following structure rhinoplasty. In this prospective randomized controlled trial, 63 patients undergoing primary structural rhinoplasty were evaluated. Patients were divided into two groups based on the use of bromelain. Patients using bromelain were classified as group 1 (<i>n</i> = 33) and those not using bromelain were classified as group 2 (<i>n</i> = 30). The degree of postoperative edema and ecchymosis was compared between the two groups. The study included 63 participants, 53 women (84.1%) and 10 men (15.9%), with a mean age of 25.86 ± 4.46 years. As a result of the between-group comparisons using oedema and ecchymosis scoring system, a statistically significant reduction in oedema was observed in the bromelain group on days 1, 3 and 5 in the right eye and on days 1, 3, 5 and 7 in the left eye. There was also a statistically significant reduction in ecchymosis in the right eye on days 3 and 7 and in the left eye on day 7 compared to the non-bromelain group (<i>p</i> &lt; 0.05). The statistically significant reduction in ecchymosis and oedema in the bromelain group suggests that the use of bromelain may be beneficial for patients following rhinoplasty.</p>
