<p>Paediatric tracheal transection, both partial and complete, is a surgical emergency. Early intervention is key to a successful outcome. The standard diagnostic and clinical approaches for airway trauma are less applicable in this population due to the distinct anatomy of the paediatric larynx and trachea. A retrospective chart audit was done of paediatric tracheal transections treated at a tertiary care center in South India over a period of two years, between January 2023 and January 2025. Three cases of paediatric tracheal transection were treated during this study period. One was a complete tracheal transection, and two were partial transections. There were considerable anaesthetic and surgical challenges in all the cases. We describe the presentation and management of these cases. Paediatric tracheal transections are challenging to manage. We present an algorithm to aid in the management of this rare presentation. Multidisciplinary working is the key to successful outcomes.</p>
