Versatility of Fascia Lata as a Reconstruction Modality in Surgical Management of Oral Submucous Fibrosis

Oral submucous fibrosis (OSMF) is a premalignant condition which results in progressive closure of mouth. Management predominantly in the realm of surgery involves fibrotomy followed by reconstruction of the defect From among the various reconstructive options available, the use of doubled layer fascia lata, a strong connective tissue, has emerged as a promising treatment option for reconstruction. To evaluate the surgical ease/difficulty, healing, effectiveness of fascia lata graft for surgical reconstruction of OSMF. Study conducted on study sample that was derived from the population of patients who presented, with restricted mouth opening of less than 20 mm. The treatment protocol includes fibrotomy followed by reconstruction of the defect with fascia lata. Predictor/Exposure/Independent Variable: The main independent variable in the study is the surgical intervention using a double-layer fascia lata graft for reconstruction following fibrotomy in patients with Oral Submucous Fibrosis (OSMF). Main Outcome Variable(s): Mouth opening (measured in mm), Cheek blowing ability, Epithelization, Inflammation, Graft uptake, Donor site healing, Duration of harvesting and surgery. Patient age and gender,Stage of OSMF (Grade III or IVa),Preoperative mouth opening, Presence of inflammation at different postoperative intervals, Postoperative pain and healing responses, Color and condition of the graft over time. Statistical software used SPSS (version 25.0; IBM Corp., Chicago, IL, USA)Student’s independent t-test to compare mean values, Chi-square test to analyse relationships between categorical variables. Significance Level: p < 0.05 was considered statistically significant. There were a total of 20 patients in our study, with a mean preoperative mouth opening of 8.10 ± 5.322 mm, increased to (40.10 ± 5.507)mm intraoperatively. After 3rd day,7th day,15th day,1 month,3 month,1 year mean mouth opening was 20.20 ± 4.867, 29.50 ± 3.028, 34.80 ± 2.348, 37.10 ± 1.524, 39.60 ± 1.578, 39.60 ± 1.578 respectively. Result were found to be highly statistically significant p < 0.001***.Cheek blowing on right side and left side gradually increases after 7 days,15 days,1 months,3 months and 1 year compared to preoperative after using fascia lata. Results were found to be highly statistically significant (p < 0.001*). The use of fascia lata grafting in treatment of OSMF holds significant promise. This innovative surgical technique offers safe, effective, and durable solution for patients suffering from OSMF.
