Use of Single Stage Infra-Auricular Flap for Paediatric Earlobe Reconstruction

Paediatric earlobe defects pose reconstructive challenges due to the need for durable contour, colour match, and volume restoration. Most established techniques are described in adult populations, with limited paediatric data.
Case ReportA 10-year-old girl underwent wide local excision of a lobular lesion with oncologically safe margins, resulting in partial earlobe loss. Reconstruction was performed using a single-stage superiorly based infra-auricular flap. The flap was raised in the subcutaneous plane, partially de-epithelialised, folded to recreate lobular bulk, and inset onto the native pinna. The donor site was closed primarily. Postoperative healing was satisfactory. At one-year follow-up, the reconstructed lobule maintained stable volume, acceptable contour, and good colour match, with no functional compromise.
ConclusionSingle-stage infra-auricular flap reconstruction is a reliable and reproducible option for paediatric earlobe defects, offering adequate bulk and tissue match without the need for cartilage or multistage procedures.
