Occult Pneumoencephaloceles Following Mastoidectomy: ‘Overlooked’ Entity?

The Intracerebral pneumatoceles, being most common and commonly associated with mastoid surgery, are sparingly reported. This paper reports observation in 2 cases wherein shortly after mastoidectomy for atticoantral disease with extensive cholesteatoma and granulations, the patients presented with severe headache and nausea. The definitive diagnosis of multiple intracerebral pneumatoceles could be established only after good resolution imaging of brain wherein the CT scan revealed multiple small air pockets in brain parenchyma without collection of pus/ fluid. The tegmen revealed erosion without any gross breach in the dura. Patient became asymptomatic following 3 days of conservative treatment. Despite majority remain asymptomatic, a post-mastoidectomy headache with minimal nausea but without signs of meningitis may raise suspicion and early intervention seems mandatory to prevent further complication. The factors predisposing to its occurrence are discussed herein and high resolution imaging is indicated.
