Translation and Validation of the Eustachian Tube Dysfunction Questionnaire-7 (ETDQ-7) to Loco-Regional Languages (Kashmiri and Hindi) of Jammu & Kashmir

To validate a Kashmiri and Hindi translation of ETDQ-7 questionnaire in ETD patients of Jammu & Kashmir and to assess the diagnostic role of ETDQ-7 by confirming with Williams test. The ETDQ-7 questionnaire was translated into Hindi and Kashmiri through a multi-step process involving forward translations by bilingual physicians, back translations by fluent English speakers, and reconciliation of the versions. Content validation was performed with 50 ETD patients to ensure clarity and usability. The validated questionnaires were administered to 150 outpatients comprising of 80 males and 70 females and M: F ratio being 1:0.8 with mean age of presentation being 23.67 ± 3.97 years. Translated ETDQ questionnaire in Hindi and Kashmiri was divided among patients as per their language convenience. Diagnosis of ETD was based on tympanic membrane findings and symptoms over the previous month, confirmed by ET function testing (William’s test). Non-normally distributed data were analyzed using the Wilcoxon W test in Jamovi software. Post-treatment ETDQ-7 scores significantly decreased across all seven questions, with mean differences ranging from 3 to 4 and p-values under 0.001. This indicated substantial and statistically significant improvements. The narrow confidence intervals confirm the precision of these results. Test-retest reliability was good (Spearman’s rho = 0.78) based on follow-up assessments. The ETDQ-7 is a valuable tool for assessing Eustachian Tube Dysfunction (ETD), demonstrating excellent sensitivity (100%) and strong diagnostic accuracy, particularly compared to the SNOT-22. Despite its focus on symptom severity and the lack of concurrent validity, it provides a robust method for diagnosis and monitoring when used alongside traditional methods.
