<p>Parapharyngeal space (PPS) tumors are rare. We present a case of parapharyngeal space pleomorphic adenoma with a symptom of otalgia and throat discomfort in an otherwise healthy patient. Here, we review the various clinical presentations, investigations and management including surgical approaches to parapharyngeal space tumor. A 37-year-old female presented with otalgia associated with throat discomfort. Oropharyngeal and nasoendoscopic examination revealed mass occupying soft palate with mass effect in nasopharynx. Imaging revealed a right parapharyngeal space mass. Transoral biopsy revealed a pleomorphic adenoma. Patient underwent open surgical excision of the said tumor. The clinical presentation of PPS mass is subtle and differential diagnosis is extensive. This broad differential can be narrowed down with radiological imaging. Management is patient-centered, based on size and location of the tumor.</p>
