<p>To determine the normal values of the aditus ad antrum diameter using computed tomography (CT) and to compare these values with the aditus diameter in patients with chronic otitis media (COM). This prospective study was conducted from 1st January 2024 to 30th April 2025 in a tertiary health care centre. A total of 405 ears from 203 adult participants were included. The study group comprised patients with unilateral or bilateral COM (mucosal type), and the control group comprised individuals with healthy ears undergoing CT scans for other reasons. Patients with cholesteatoma, ear malignancy, or congenital ear anomalies were excluded. High-resolution CT (HRCT) of the temporal bone (1&#xa0;mm cuts) was performed. The narrowest inner diameter of the aditus ad antrum was measured on both axial and coronal planes. Data was analyzed using an independent sample t-test, with statistical significance set at <i>p</i> &lt; 0.05. The mean aditus diameter was significantly smaller in ears with COM compared to healthy ears on both axial (1.636 ± 0.49&#xa0;mm vs. 1.936 ± 0.438&#xa0;mm) and coronal planes (1.937 ± 0.44&#xa0;mm vs. 2.339 ± 0.40&#xa0;mm) (<i>p</i> &lt; 0.001). In healthy ears, males had a significantly larger aditus diameter than females on the coronal plane (2.396&#xa0;mm vs. 2.264&#xa0;mm, <i>p</i> = 0.011). In both males and females, the aditus diameter was significantly narrower in diseased ears compared to healthy ears (<i>p</i> &lt; 0.05). The presence of myringosclerosis was not associated with a statistically significant narrowing of the aditus diameter (<i>p</i> &gt; 0.05). A narrowed aditus ad antrum diameter is significantly associated with chronic otitis media. CT measurement, particularly on the coronal plane, is a valuable tool for assessing the aditus, and its narrowing may be an associated anatomical factor in the pathogenesis of COM by impairing middle ear ventilation.</p>
