Post Covid Mucormycosis, Our Surgical Experience and Management at Government General Hospital, Kurnool

India was affected a lot with the worldwide pandemic COVID-19 caused by SARS-COV 2. Incidence of Mucormycosis in India, is 0.14 per 1000. The worldwide incidence of mucormycosis, appears to be increasing particularly in oncological patients and those with diabetes. Despite the aggressive surgical and antifungal therapy, overall morbidity remains high. to know the stage of disease presentation, level of surgical management based on the stages of disease and the duration of medical management with liposomal amphotericin b and posaconazole. A randomized retrospective study of 100 patients who underwent treatment for Rhino-Orbital-Cerebral-Mucormycosis (ROCM) over a period of 1 year. Mucormycosis was mainly seen in the age group of 41–50 years and males were commonly affected. Only 1 patient was < 18 years with Type 1 Diabetes Mellitus. The majority of the patients had symptoms for less than 2 months. Diabetes, hypertension, covid exposure, and chronic kidney disease were the major risk factors. The majority of the patients were in stage 3 and immune-compromised. Patients were managed with liposomal amphotericin B, posaconazole and surgical procedures including FESS, Denkers and maxillectomy. The recurrence rate was minimal. No case of mortality was seen during the study period. Early suspicion and diagnosis of ROCM are crucial to prevent complications. Uncontrolled diabetes mellitus is the leading factor causing complications and poorer outcome. Effective reduction of morbidity and mortality requires timely management using nasal endoscopy, radiographic studies, endoscopic surgical debridement, and intravenous administration of antifungal drugs to address the underlying cause.
