<p>Retropharyngeal lipomas are rare benign tumors of the head and neck region. Symptoms are caused due to increasing pressure on surrounding structures. We report a 64-year-old male with snoring and progressive dysphagia. Examination revealed a bulge in the left side of the posterior pharyngeal wall. CT and MRI neck showed a large fat-containing lesion in the left postero-lateral retropharyngeal space, extending from the skull base to C2, suggestive of lipoma. The mass was excised transorally, and histopathology confirmed lipoma. Retropharyngeal lipomas are a rare cause of airway obstruction and dysphagia. Due to their anatomical site, diverse symptomatology, and diffuse growth pattern, diagnosis may be delayed. Imaging is essential for diagnosis, and surgical resection offers a curative treatment with excellent outcomes.</p>
